CarePartners is looking for a nurse (LVN or RN) to join our Dementia Day Center Team. The Dementia Day Center Nurse is a critical role in Day Center operations, providing care to our clients and support to their families. Part time or full time options available.
The Nurse provides daily direct health care to Dementia Day Center participants including medication administration, required healthcare documentation and reporting, emergency first aid, and behavioral interventions. As a member of the management team, the Day Center nurse guides team members in effective dementia behavior interventions, provides education and counseling to Day Center family members, and manages relationships with both incoming and current Day Center families.
Ideal candidates must be able to communicate effectively one-on-one and in small groups and work most efficiently in team settings. They must also exhibit compassion and inclusivity of individuals at all stages of dementia. Individuals must be an LVN or RN with an active license in Texas.
Other requirements include:
- Strong skills in medical and healthcare documentation, including the use of electronic health record systems.
- Sensitivity and non-judgmental attitude regarding social, cultural and economic status.
- Flexibility in roles as the needs of participants and families change.
- Clear and compassionate communication skills.
- Experience in dementia care, long term care, or aging services preferred.
- Bilingual (Spanish) preferred.

Care Team Coordinator

CarePartners is looking for an outgoing, service-oriented individual to join our team as a Care Team Coordinator (CTC). The CTC is a part of a team dedicated to providing care, education and support to older adults and their families across the Greater Houston area.
The Care Team Coordinator (CTC) acts as the go to resource for our Gathering Places, volunteer-led group activity and respite programs for individuals with dementia. The CTC leads and supports the operations of the Gathering Place sites and volunteers by providing onsite supervision and assistance, on-going volunteer training and education, and resource identification and development. Additionally, the CTC assists in the recruitment of new Gathering Place partner sites, volunteers, and attendees through one-on-one engagement and outreach activities.
Ideal candidates must be able to communicate effectively one-on-one and in small groups and are able to work independently and in team settings. They also need reliable transportation and willingness to commute around the Greater Houston area to various sites (will be compensated for travel).
Other requirements include:
- Strong Computer skills using Microsoft Office (Word, Excel, Outlook, PowerPoint) is required as well as proficiency in utilizing online communication tools like Zoom and Teams.
- Sensitivity and non-judgmental attitude regarding social, cultural and economic status.
- The ability to exhibit flexibility in work hours as some meetings and events occur during evenings and weekends.
- Experience in social services, dementia care, aging services, or similar industry preferred.
- Bilingual (Spanish) preferred.

Personal Care Assistant

The Personal Care Assistant assists with specialized activity programming and personal care for persons with dementia in the CarePartners Dementia Day Center. Responsibilities include using person-centered care to assist with Day Center clients’ personal care needs and ensuring clients’ involvement in meaningful and enjoyable activities. Experience and Requirements:
- Must be at least 18 years old; high school graduate or equivalent
- Experience working with older adults with dementia
- Training or experience in activity programming and ability and willingness to provide personal care assistance to older adults
