Chief Development Officer

Reporting to the CEO and working closely with the Board of Directors, the CDO secures funding from individuals, corporations, foundations, and government entities to support ongoing operations and special projects. Care Consultant

Are you passionate about supporting family caregivers and making a meaningful impact in the lives of those affected by dementia? CarePartners is looking for a dedicated Care Consultant to provide guidance, education, and resources to caregivers navigating the challenges of dementia care.

As a Care Consultant, you will be a trusted resource for families, offering individualized support through assessments, care planning, and referrals to vital services. You will help caregivers develop strategies to improve their well-being while ensuring their loved ones receive the best possible care.

Key Responsibilities:

  • Provide one-on-one consultations, education, and support to caregivers of people with dementia
  • Conduct caregiver assessments to identify needs and develop personalized care plans.
  • Guide families through care options, community resources, and long-term planning.
  • Lead educational workshops and facilitate support groups to empower caregivers.
  • Support the development and implementation of caregiver support programs, including support groups, educational workshops, and caregiver conferences.
  • Collaborate with community organizations to expand access to caregiver support.


Who We’re Looking For:

  • Bachelor’s degree in social work, psychology or related work experience.
  • Experience in care consultation, dementia care, case management, or family support services.
  • Strong communication and counseling skills with a compassionate approach.
  • Ability to work independently and collaboratively in a team setting.
  • Proficiency in electronic health records and documentation.
  • Bilingual (Spanish) preferred to better serve diverse communities.
