CarePartners bridges the gap between healthcare and social services by offering programs across Greater Houston which support caregivers’ physical and mental health while accommodating their loved ones’ needs, interests, and abilities.
Dementia Day Center: Licensed adult day center for individuals with mild to moderate-late dementia, providing specialized care and physical, mental, and social stimulation in a nurturing environment.
The Gathering Place: Half-day activity programs that provide socialization and engagement for individuals with mild to moderate memory loss and offer their family caregivers time to recharge their batteries and attend to their own health, errands, or personal business.
Common Ground: Peer support groups that offer connection and resources to family caregivers.
Caregiver Consultation: Ongoing one-on-one support, guidance, and action-planning for family caregivers by a licensed master-level social worker.
Caregiver Education: Workshops and conferences that provide education and resources to caregivers at any stage of the caregiving journey.
Most programs are offered at no cost, thanks to the generosity of our partners and donors.
The heart of our mission is to bring caring volunteers together with families in need of support. Programs are complimentary to caregivers and those suffering from memory loss or other challenges of aging.
