CarePartners programs are located across the Greater Houston area. We collaborate with more than 60 different partners and over 1,200 volunteers to deliver high-quality programming for older adults, individuals with dementia, and family caregivers.

Our community-based programs offer many opportunities for connection:

Caregiver Education: Workshops and conferences that provide education and resources to caregivers at any stage of the caregiving journey.
Common Ground Support Groups: Peer support groups that offer connection and resources to family caregivers.
Gathering Place Program: Half-day activity programs that provide socialization and engagement for individuals with mild to moderate memory loss and offer their family caregivers time to recharge their batteries and attend to their own health, errands, or personal business.
Second Family Program: In-home program in which volunteers visit homebound people to provide companionship and opportunity for their caregivers to step away to attend to their own needs.
Dementia Day Center: Licensed adult day center for individuals with mild to moderate-late dementia, providing specialized care and physical, mental, and social stimulation in a nurturing environment.

We understand that caregivers’ responsibilities sometimes do not allow them to participate in-person. We offer a menu of services that can be accessed virtually.

Care Consultation: Trained care consultants provide caregivers with ongoing, wide-ranging one-on-one support via telephone or in-person, from accessing resources to emotional support.
Virtual Common Ground Support Groups: A general caregiver support group and a grief support group are offered virtually every week.
Caregiver Education Webinars: Education webinars bring important topics to the caregiver, ranging from legal and financial information to how to talk to your loved one’s doctor.
Caregiver Resource page: An overview of relevant topics can supply caregivers with vital tools for their toolbox. It is a 24-hour resource at your fingertips. Don’t know where to start? Take the caregiver assessment for an individualized list of resources.
